6. Items 5 & 11 require you to enter the full names of each biological parent.
7. For Items 6 & 12, list all previous legal names used by each biological parent.
8. Check the gender boxes for each biological parent in Items 7 & 13.
9. Input the dates of birth for each biological parent in Items 8 & 14 using mm-dd-yyyy format.
10. Complete Items 9 & 15 by entering places of birth for each biological parent. Include city and state if born in the US or city and country if born abroad.
