  3. Fill in the PSSYF Agency Name and Provider ID # to identify your agency.
  4. Enter the youth's information, including their Identified Youth Name, Date of Birth, DCF Family Case # (if applicable), Social Security Number, and Unique ID/Medicaid Number.
  5. For Authorization, provide the Reason for Services and specify the Start date of services.
  6. If completing a Discharge, indicate the Reason for Discharge and who is completing it. Fill in their name and Discharge Date.
  7. Obtain DCF District Approval by having the District Director sign and print their name along with the date.
  8. Finally, ensure that all completed forms are emailed to the specified addresses for processing.

Soldiers should use a Personnel Action Request (PAR) to request stabilization no earlier than March 1 of the students freshman year and no later than the start of the students sophomore year (applying between March and September). This application window also applies to students in year-round schooling programs.
In order to stabilize a Soldier serving OCONUS past their current DEROS, the unit must, first submit a Foreign Service Tour Extension (FSTE) to change the Soldiers DEROS to the desired Restriction End Date. Once the FSTE is approved, the unit may apply the ASGNCONS ACKDCL for CD stabilization.
High School Junior/Senior Stabilization Programs in each military branch provide stability to armed forces members who have children in their senior (and/or junior) years of high school by allowing them, ing to each branchs policies, to remain in their current assignment.
HRC authority to waive approved stabilization: Restriction Code SBCREW stabilization may be waived at the Director, FSD/TADD level, when needs of the Army require. Approval authority may be delegated to the Division Chief (O6) level.
The stabilization period is dedicated to the gradual adjustment of stimulation parameters and medication. It is normally completed within the 36 months after surgery, but complicated cases may take up to a year.
