(a) Form CA7 is used to claim compensation for periods of disability not covered by COP. (1) An employee who is disabled with loss of pay for more than three calendar days due to an injury, or someone acting on his or her behalf, must file Form CA7 before compensation can be paid.
Employees who are eligible for wage replacement benefits pursuant to the Workers Compensation Law receive no wage replacements for the first seven calendar days of disability (which is the Workers Compensation Law initial waiting period), unless the disability extends beyond 14 calendar days.
To be eligible for COP (45 days) the employee, or someone acting on their behalf, must file the CA-1 within 30 days following the injury. They must also provide medical evidence in support of the disability within 10 days of submission of the CA-1.
When an injured worker has permanent loss of use of certain body parts or organs, s/he may request a schedule award by submitting a CA-7 Claim for Schedule Award and an impairment rating completed by her/his treating physician.
