Begin by filling out the VESSEL IDENTIFICATION section. Enter the name and port of your vessel, registration number, year/make, length, type, hull color, and any unique features.
In the PROPULSION section, specify the primary and auxiliary engine horsepower and fuel capacity.
Complete the COMMUNICATIONS section by checking all onboard equipment and providing details for each radio type and frequency monitored.
Fill out the NAVIGATION section by checking all navigation tools you have on board.
In SAFETY & SURVIVAL, list personal flotation devices (PFDs), visual distress signals, ground tackle information, audible distress signals, medical supplies, and other gear available.
Provide details about PERSONS ON BOARD including operator information and passenger details such as names, ages, sexes, and any medical conditions.
Lastly, complete the ITINERARY section with dates, times, locations of travel stops, reasons for stops, and check-in times. Attach a supplemental itinerary if needed.
