1. Click ‘Get Form’ to open the VECHS Waiver in the editor.
2. Begin with Section (A). If you have been fingerprinted before, enter the name of the organization that conducted your fingerprinting, along with the year and their address details.
3. In Section (B), indicate whether you have 'have' or 'have not' been convicted of a crime. If applicable, provide details about your conviction in the text field provided.
4. Proceed to Section (C). Check the box that corresponds to your status: Employee, Volunteer, or Contractor/Vendor. Ensure you type your full legal name and include your VU NET ID in parentheses.
5. Fill in your current home address, city, state, zip code, and date of birth. Then move to the digital signature box.
6. To sign digitally, click into the signature field. If you already have a digital signature created, select it; otherwise, create a new one by following the prompts.
7. After signing, rename the document using your VU NET ID and place 'VECHS' in the subject line before emailing it to bcoconsents@vanderbilt.edu.
