- In Section (B), indicate whether you have 'have' or 'have not' been convicted of a crime. If applicable, provide details about your conviction in the text field provided.
- Proceed to Section (C). Check the box that corresponds to your status: Employee, Volunteer, or Contractor/Vendor. Ensure you type your full legal name and include your VU NET ID in parentheses.
- Fill in your current home address, city, state, zip code, and date of birth. Then move to the digital signature box.
- To sign digitally, click into the signature field. If you already have a digital signature created, select it; otherwise, create a new one by following the prompts.
- After signing, rename the document using your VU NET ID and place 'VECHS' in the subject line before emailing it to bcoconsents@vanderbilt.edu.
