Begin by completing Part I. Enter your first, middle, and current last names as listed on the marriage certificate in fields 1A, 1B, and 1C respectively. If your birth last name differs from your current last name, include it in field 1D.
Provide the date of marriage in MM/DD/CCYY format in field 2 and specify the county where the license was issued in field 3.
Continue filling out fields 4A to 4D with names as they appear on the original certificate.
In Part II, list the item number to be corrected in field 5. Then, enter the incorrect information from the original record in field 6 and provide the corrected information in field 7.
State your reason for correction in field 8. Ensure both parties sign and date their respective sections (fields 9A and 10A) before submission.
Finally, return the completed form along with any required fees to the appropriate registrar's office.
