8. For Field 8, specify the origin criterion by entering 'P' for wholly produced items or 'Y' followed by a percentage for partially produced items.
9. Enter gross weight or other quantity in Field 9 as shown on your Commercial Invoice.
10. Identify accompanying invoices by number and date in Field 10.
11. Complete Fields 11 and 12 with certification details from a recognized authority if applicable.
12. Finally, sign and date your declaration in Field 13 to confirm accuracy before submission.
