7. Fill out information regarding the fail to pay incident, including fuel type, number of litres, value of fuel, and any relevant CCTV details.
8. Describe both the offending vehicle and its driver/passenger thoroughly using all available fields for accurate reporting.
9. Once completed, fax this form to 02 4353 4948 as instructed. A Police Report will be generated and returned with an Event Number.
