The DA Form 3749 is an official United States Army form used for the issue or turn in of equipment or weapons. Specifically, this form acts as an equipment receipt, providing proof of issue and authorization of the equipment held by individuals.
DD Form 1380 promotes Department of Defense goals of capturing documentation of pre-MTF medical interventions at the POI using a MIST format (Mechanisms of injury; Injuries; Signs Symptoms; and Treatments). It is designed for use by all first responders, including non-medical personnel.
Equipment Receipt (DA Form 3749, Weapons Card) for 24 hours or less. Over 24 hours requires entry on the Weapons Control Log.
Prepare DA Form 1380 by the last day of each month, to cover the following: a. Nonunit Reserve training, other than Army Extension Courses. b. Equivalent duty or other appropriate duty performed by Soldiers assigned to USAR units, other than such training performed with the unit of assignment.
