2. Begin by selecting the sport for which you are submitting eligibility. Fill in your school name and address, ensuring all details are accurate.
3. In the certification section, provide signatures from both the coach and superintendent's designee. Make sure to include their contact numbers.
4. List each student alphabetically in the designated fields. Include their birth date, date of first enrollment in 9th grade, and any relevant enrollment dates for this year.
5. If applicable, mark an 'X' next to students who have participated in another district previously and indicate any waivers approved by the superintendent.
6. Before submission, check that all required signatures are secured and that all columns are filled out completely as per the checklist provided at the end of the form.
