Begin by completing the Civil Cover Sheet (FL-NC-100) with your personal information, ensuring all details are accurate and legible.
Next, fill out the Petition for Change of Name (FL-12-982f). Clearly state your reasons for the name change and include any required details.
If applicable, complete the Supplemental Forms for each family member whose name is changing. Ensure that each page is initialed by the petitioner(s).
Once all forms are completed, print three copies. Sign where indicated in front of a notary or court clerk as required.
File your documents with the court and request a 'file stamp' on one copy for your records. Follow up by setting a hearing date with the clerk.
