  9. List any additional phone numbers you wish to port from the same provider, ensuring they are included on the attached invoice.
  10. Finally, review all entries for accuracy before submitting. Attach a signed copy of this form along with a recent invoice from your current provider.

For this, an SMS is to be sent from the mobile number which is to be ported, the word PORT (which shall be case-insensitive, i.e., it can be port or Port etc.) followed by a space and the ten-digit mobile number which is to be ported, to 1900.
You need a Unique Porting Code (UPC) and valid Proof of Identity (POI) and Proof of Address (POA) document such as Aadhaar, Voter ID, Passport, or Driving License. Alternatively, you can also use the eKYC facility.
