  6. Enter the phone number you wish to port and include a PIN code if you're transferring a cell phone number.
  7. Select your desired rate plan from the options provided. If no selection is made, it will default to 'Per Minute'.
  8. If you have an existing Anveo phone number that you want to keep, specify which one should be replaced.
  9. List any additional phone numbers you wish to port from the same provider, ensuring they are included on the attached invoice.
  10. Finally, review all entries for accuracy before submitting. Attach a signed copy of this form along with a recent invoice from your current provider.
