Click ‘Get Form’ to open the dtsc lines application in the editor.
Begin by entering the 'Name of Legal Entity' in section 1. This should reflect your business's legal name, whether it's an individual, partnership, corporation, or LLC.
In section 2, if you are renewing or reinstating your registration, input your existing Transporter Registration Number. For new applications, leave this blank.
Proceed to section 3 and provide your 'Business Name' as it will appear on the registration. Ensure consistency across all documentation.
Select the appropriate 'Application Type' in section 4 by checking one of the boxes for New, Reinstatement, Renewal, or Midyear Amendment.
Fill out sections 5 and 6 with your complete business address and mailing address if different. Accuracy is crucial for correspondence.
In section 7, enter your contact numbers including telephone and fax numbers along with an email address for communication purposes.
For section 8, list all Transporter Identification Numbers used by your business. If you have multiple numbers, include them on a separate sheet if necessary.
Complete sections 9 and 10 by providing any additional business addresses and selecting the ownership type of your business.
Finally, ensure that an authorized representative signs in section 11 using blue ink. Include their printed name and title along with the date of signing.
