Open the PDF document and select Sign from the top tools bar. Alternatively, from the All tools menu, select Fill Sign. Create your signature and initials if not already done.

Open the PDF document or form in Acrobat or Acrobat Reader, and select Fill Sign from the Quick action toolbar. From the signature dialog, select the cross (X) sign beside the previously added signature. Select the X sign. To add a replacement signature to the document, select Add Signature.
In the signature Properties window click Show Signers Certificate button. Following screen will be displayed once you click on it. Once you click on OK button, following window will be displayed.
