The document has been reviewed by two colleagues and revisions have been accepted or rejected. To ensure no remaining revisions, open the reviewing pane. Inspecting the document under the file tab can also check for hidden comments or changes by selecting "inspect document." Choose to check for comments, revisions, versions, and annotations. Remove other check boxes and click inspect to confirm no hidden items were found, ensuring the document is ready for publishing.

If you go to the document properties of a PDF file (control or command d), if the proper metadata is available, it will list the creation date and time and modified date and time. This can help you determine if a pdf file has been modified since creation.
