STANDARDIZED MORTALITY RATIO (abbreviated SMR) is the number of observed deaths in the study population divided by the number of expected deaths (calculated from indirect adjustment) and multiplied by 100 (Lilienfeld Stolley, 1994; Last, 2001).
Direct age-adjustment (or age standardization) is the same as calculating a weighted average. It weights the age-specific rates observed in a population of interest by the proportion of each age group in a standard population (Lilienfeld Stolley, 1994).
The SMR may be quoted as either a ratio or a percentage. If the SMR is quoted as a ratio and is equal to 1.0, then this means the number of observed deaths equals that of expected cases. If higher than 1.0, then there is a higher number of deaths than is expected.
Definition: The age-standardized mortality rate is a weighted average of the age-specific mortality rates per 100 000 persons, where the weights are the proportions of persons in the corresponding age groups of the WHO standard population.
Standardized Mortality Ratio (SMR) = (Observed Deaths / Expected Deaths) SMR = (481 / 430.98) = 1.12. Excess Deaths = (Observed Deaths - Expected Deaths) Excess Deaths = (481 - 430.98 = 50.02 or 4.5 deaths per year (50.02 / 11)
That is, for each age group, ASDR = deaths in age group estimated population of that age group 100,000. This is called the direct method of standardization.

Multiply the number of people in each age group of the population(s) of interest by the age-specific mortality rate in the comparable age group of the reference population. Sum the total number of expected deaths for each population of interest.
