Welcome back to my channel Adventures in America. Today, I will discuss how to fill out the CDC Attestation form. This form is required for all passengers to the US and has been updated with a new version that expires on 6/30/2022. Starting from April 14, parents or guardians of children under two do not need to complete the form for them. There are two sections: one for US citizens and permanent residents, and one for non-US citizens above two years old. You can find the form on the CDC website, but I will demonstrate how to fill it out in this video. The new form has five pages.
