What is not a HIPAA identifier? De-identified data or health information that cannot be used to identify an individual or provide a reasonable base to identify them is not a HIPAA identifier.
Patient names. Geographical elements (such as a street address, city, county, or zip code) Dates related to the health or identity of individuals (including birthdates, date of admission, date of discharge, date of death, or exact age of a patient older than 89)
Examples of PHI include: Any dates (except years) that are directly related to an individual, including birthday, date of admission or discharge, date of death, or the exact age of individuals older than 89.
Identifying numbers: Social Security number (full or partial) Date of birth (Please note that the full date is an identifier; however, if you want to collect an individuals age, year of birth, and/or month of birth, this information is not considered an identifier.)
A HIPAA authorization is a detailed document in which specific uses and disclosures of protected health are explained in full. By signing the authorization, an individual is giving consent to have their health information used or disclosed for the reasons stated on the authorization.
What does the identifier dates, but not years mean? This means that any date directly related to an individual (birth date, admission date, discharge date, etc.) is considered as PHI under HIPAA except the year.
