To fill up a form on PDF on your mobile device, first download Microsoft 365 Office app if you don't have it already. Open the app and click on the upper right corner icon, then select files app. Open the PDF file you want to fill up, click on the edit button at the bottom of the screen, and tap the screen to start editing and filling up the form.
