The DNR guides Emergency Medical Service (EMS) providers and can give EMS permission not to perform cardiopulmonary resuscitation (CPR), whereas a POLST might include a DNR instruction regarding CPR, but provides more instructions regarding additional medical interventions.
At any time, the patient or the patients health care decision-maker may rescind consent to the DNR directive, regardless of how long it has been in place. They must simply ask any member of the health care team to remove it, which immediately invalidates the directive.
The most accurate form of identification for patients outside of licensed facilities is a medallion or bracelet attached to the patient. Use of such a medallion should never make the patient uncomfortable and should always be optional. Every local EMS system should accept an EMSA approved DNR bracelet or medallion.
A DNR order can be reversed if it becomes medically appropriate. If a patient changes his or her mind about the DNR order, this should be discussed with the attending physician. Even if the DNR order is written, this does not mean that other lifesaving treatments, such as antibiotic administration, will be withheld.
The only instance in which a family member can revoke a DNR is when that same family member is also the patients healthcare agent. If the patient authorized that family member to serve as their agent through their Medical Power of Attorney document, then that family member does have the power to cancel the DNR.
At many hospitals, the policy is to write a DNR order only with patient/family agreement. If there is disagreement, every reasonable effort should be made to communicate with the patient or family. In many cases, this will lead to resolution of the conflict. In difficult cases, an ethics consultation can prove helpful.
The DNR must be signed by the patient or by the patients legally recognized health care decision maker if the patient is unable to make or communicate informed health care decisions.
