There are three very basic things you should do every time a patient codes. Check Pulse. Get on the chest immediately if you dont get a pulse. Give Them Oxygen.
Full code status is recommended for individuals who are healthy and do not have multiple medical conditions. Limited code means that only specific resuscitation procedures will be used during a medical emergency.
If a patient has a Do Not Resuscitate (DNR) order on file, the hospital staff must respect its legal boundaries. That usually precludes issuing a code blue. Patients with a DNR typically do not receive any cardiopulmonary resuscitation (CPR) efforts or any form of advanced cardiac life support (ACLS).
These patients have legal orders stating they do not wish to receive cardiopulmonary resuscitation (CPR) or advanced cardiac life support (ACLS). When you encounter a DNR patient in cardiac or respiratory arrest, you dont perform these measures, and you shouldnt call code blue.
A code blue in adults should be called immediately for any patient whos unresponsive, apneic, and/or pulseless. Under American Heart Association (AHA) guidelines, calling for help and initiating CPR should be done simultaneously.
The American Heart Association in 2005 moved from the traditional do not resuscitate (DNR) terminology to do not attempt resuscitation (DNAR). DNAR reduces the implication that resuscitation is likely and creates a better emotional environment to explain what the order means.
A DNR order does not mean do not treat. Rather, it means only that CPR will not be attempted. Other treatments (for example, antibiotic therapy, transfusions, dialysis, or use of a ventilator) that may prolong life can still be provided.
DNACPR is sometimes called DNAR (do not attempt resuscitation) or DNR (do not resuscitate) but they all refer to the same thing. DNACPR means if your heart or breathing stops your healthcare team will not try to restart it. A DNACPR decision is made by you and/or your doctor or healthcare team.
A DNR/DNAR order is applicable only in the event of a cardiopulmonary arrest and does not refer to withholding or discontinuing other supportive therapies that may be in place (such as the ongoing administration of cardiotonic drugs, an endotracheal tube, etc.).
There are currently two types of DNR orders: 1) DNR Comfort Care, and 2) DNR Comfort Care - Arrest. Upon the issuance of either order, standard forms of identification are provided for in OAC rule 3701-62-04.
