Go to Review Protect Document. Under Security, you can select whether to enter a password to open the document, modify the document, or both. Enter each password again to confirm. Click OK.
Select Close Header and Footer or double-click anywhere outside of the header or footer area to exit.
In the table, right-click in the row that you want to repeat, and then click Table Properties. In the Table Properties dialog box, on the Row tab, select the Repeat as header row at the top of each page check box. Select OK.

With the text box selected, choose Text BoxPositionMore Layout Options and check the Lock anchor option. This will keep the text box anchored to the paragraph at the top of the page. If the paragraph at the top of the page moves to another page, the text box will move with it.
Open the template that you want to help protect from changes. On the Developer tab, in the Protect group, click Protect Document, and then click Restrict Formatting and Editing. In the Protect Document task pane, under Editing restrictions, select the Allow only this type of editing in the document check box.
(1) Enter a name for this content control in the Title box; (2) Check the option of Content control cannot be deleted; (3) Check the option of Contents cannot be edited; (4) Click the OK button.
On the Review tab, in the Protect group, click Restrict Editing. In the Editing restrictions area, select the Allow only this type of editing in the document check box. In the list of editing restrictions, click No changes (Read only).
