Add a standard or customized header or footer Go to Insert Header or Footer. Choose from a list of standard headers or footers, go to the list of Header or Footer options, and select the header or footer that you want. When youre done, select Close Header and Footer or press Esc.
On the Insert tab, in the Header Footer group, click Header or Footer. Click Edit Header or Edit Footer. On the Headers Footers tab, in the Navigation group, you can find the Link to Previous option available, and you can turn if On or Off.
Select Layout Breaks Next Page. Double-click the header or footer on the first page of the new section. Click Link to Previous to turn it off and unlink the header or footer from the previous section. Note: Headers and footers are linked separately.

Insert Page X of Y Go to Insert Page Number. Do one of the following: Select Current Position if you have a header or footer. Scroll until you see Page X of Y and select a format. Select Close Header and Footer, or double-click anywhere outside the header or footer area.
Add the file name, date, author or other document properties to a header or footer Double-click the header or footer . Select Document Info, and choose the information you want. Select Close Header and Footer or press Esc to exit.
Select the text and then click on Insert, then Link (or just press Ctrl+K after selecting the text). On the Insert Link dialog select Web link, and then enter the address of your Google form. Good luck!
