You will need to fill out a W-9 form if you: Classify yourself as an independent contractor or freelancer. Are not a full-time employee of the business. Will be paid more than $600 for work provided to the business.
In general, income that results from a W-9 arrangement is not subject to IRS withholding. Rather, it is the payees responsibility to claim the income on his or her tax return, and to pay any appropriate taxes.
On line 3, select just ONE box. Check the appropriate box for the U.S. federal tax classification of the person whose name is entered on line 1. Line 4 is for exemption codes. Exemption codes are for those payments that are exempt from backup withholding.
You must write your name as it is shown on your tax return. If you use a middle initial on your tax return, use one on Form W-9. The next box asks for your business name. If you do not have a business name, you may leave this box blank.
If you are a sole proprietor or single-member limited liability company (LLC), you should enter your own name on line 1 as well.
