This Amended Order went into effect for flights departing to the US from a foreign country at or after 12:01am EST (5:01am GMT) on December 6, 2021, and was published in the Federal Register on December 7, 2021. All travelers regardless of citizenship or vaccination status must complete the CDC attestation confirming they meet U.S. entry requirements. If youre a non-U.S. citizen, you must also have proof of vaccination against COVID-19 to fly, unless you qualify for an exception.
One attestation form must be filled out for each passenger age 2 years or older who is not a U.S. citizen, U.S. national, lawful permanent resident, or an immigrant (Covered Individual), and who is seeking to enter the United States by air travel.
