To use the scannable 2D barcode on Form RP-5217, download and complete it using docHub. Download the form in Firefox by clicking the button at the bottom left. Make sure not to open the form in the browser, as it won't have the barcode. Check the download status by clicking the menu icon, then downloads. Open the form in docHub by right-clicking, selecting Open with, then docHub Reader. Save the form each time you make changes.
