Its greyed out because it needs somewhere to insert the page number. You have no insertion point active in the document to insert the page number. Click in the body of the document in a text box or other text entry area, then try selecting Insert Page Number.
It is because the text is within a field. To confirm that it is a field, press Alt+F9 to toggle on the display of field codes and see if your text is enclosed within a pair of field delimiters { }. Use Alt+F9 again to toggle off the display of the field codes.

Most likely, you have you document set up with different first page headers and footers. Open the header/footer of the first page, select the page number and copy and paste it to the header or footer of the subsequent pages.
