In order for the scannable 2D barcode to function on Form RP-5217, you must download and complete the form using docHub. To download Form RP-5217 using your Firefox browser, select the Download Form RP-5217-PDF button located at the bottom left of the screen. Note: If your settings allow, Mozilla Firefox may try to open Form RP-5217-PDF in your browser window when complete, but this will not have the necessary 2D barcode. Do not use a version of the form without a 2D barcode. To check the status of the download, select the menu icon located in the top right corner of the screen. Then select the downloads option. When the download is complete, select the Show in folder icon to open your computers downloads folder. Form RP-5217 should be the most recent item in that folder. Right-click on the form and select Open with. Then select docHub Reader. The form is now open and ready to be filled out. Remember to save the form to your computer each time you change it.
