To add a check box field: Open the PDF in Nitro Pro. On the Forms ribbon, in the Form Fields group, click Check Box. On the page, drag the pointer where you want the field to appear. If editing an existing radio buttons, place a cursor over it and click a right mouse button. Select "Properties..." from the popup menu. Select the "Options" tab on the "Radio Button Properties" dialog. Select a button style.
