hi there jamie here today at teachers tech hope youre having a great day today today i want to show you how you can make a fillable form in microsoft word so what i mean by this you can go ahead and create a form that for example looks like this where you can be entering text and places you can be having drop downs you can be having check boxes you can even have a place to upload an image to you can even do a calendar for an order date or whatever you need to order it from and so theres all these different options in creating these forms and what i like is afterwards when youre done it you can lock it down so if youre sending this out to people they cant type anywhere else and they cant move anything to change it up and then they can go ahead and send it back to you or print it off so today on teachers tech i want to show you how you can create these fillable forms in microsoft word so im going to start from scratch and building this form with a bla

In your form, click on the question you want to add a dropdown list to or add a new question by clicking the plus sign in the floating menu on the right. Click the box in the upper right corner of the question field. Then select Dropdown. Now you can add items for your dropdown list by clicking Add option.
Google Forms has a dependent functionality in its dropdown feature where the user will be diverted to a section based upon selecting an option from the dropdown list. Google Forms cannot connect one or more dropdowns as the user selects options from each dropdown list.
From the DATA tab, select Data Validation. Click Data Validation in the drop-down list. In the dialog box, select List from the Allow drop-down menu. In the source field, type the choices youd like your drop down menu to include, separated by commas.
Place your cursor in the document where you want to add the drop-down list. Go to the Developer tab and click the Drop-Down List Content Control button. Youll see the control pop into your document as a box with Choose an item. inside. You can then move on to setting up the list.

Make the dependent drop down. Set up the first drop down. Configure the second drop down. Set up the third drop down.
You can create that by clicking the dropdown in the upper right-hand corner of the form that currently displays Multiple choice. From the dropdown list, select Multiple choice grid.
Choose Question - first go to menu item Add-ons -- Dynamic Fields -- Create Mapping and select a question where you like to create a mapping to insert values dynamically. Please be aware that only questions of type Drop-down, Multiple Choice, Grid, List and Checkbox are displayed for selection.
