The HTML is used to define a Radio Button. Radio Buttons are used to let the user select exactly one option from a list of predefined options. Radio Button input controls are created by using the input element with a type attribute having value as radio.
You group radio buttons by drawing them inside a container such as a Panel control, a GroupBox control, or a form. All radio buttons that are added directly to a form become one group. To add separate groups, you must place them inside panels or group boxes.
To create a radio button in HTML, use the element with the type radio.
Adding a Radio Button Group to an HTML Form Go to Insert Form Items Radio Button Group. This opens the Insert Radio Button Group window. Enter a name for the group. This will appear in your form results. Enter your radio button items. Click Add to add more item-value pairs to your list. Click OK.

When should you use radio buttons or checkboxes? This relies entirely on what question you are asking. If you want your users to select multiple options, use checkboxes. If you want your users to select only one option, use radio buttons (or another alternative).
Checkboxes and radio buttons are elements for making selections. Checkboxes allow the user to choose items from a fixed number of alternatives, while radio buttons allow the user to choose exactly one item from a list of several predefined alternatives.
/ structures are used for grouping form controls that are related in some way. While in complex forms they can be used to group all kinds of form controls, groups of radio buttons and checkboxes should always be grouped by them.
As Gav commented above, radio buttons must have an ID, and name. You must set your Label up with the attibute for, which matches the ID of the radio button. Even though your input is nested inside the label, you will still need an ID for each. The name attribute can be common.
