Open the form or report in Design view by right-clicking the form or report in the Navigation Pane, and then clicking Design View. On the Design tab, in the Controls group, click Text Box. Position the pointer where you want the text box to be placed on the form or report, and then click to insert the text box.
You can use a text form field to collect basic text information such as a name or address. Alternatively you can also use text form fields to request other types of data such as numbers, dates, times, phone numbers, zip codes, percentages etc. Text form fields can also be used to perform calculations.
Place the cursor in your document where you want to insert the field. From the Insert tab on the Ribbon, in the Text group, select Quick Parts and then select Field . In the Field dialog box, from the Field names list, select the field. You can filter this list by selecting Categories .
On the Insert tab, in the Text group, click Text Box, and then click Draw Text Box. Click and drag in the message body to draw a text box the size that you want. To add text, click inside the box and type or paste your text.
Click in your Word document wherever you wish to insert a Form Field. On the Legacy Forms menu click the first icon to insert a Form Field. Right-click on the Form Field and select Properties. Then provide a name for the field in the Bookmark section.
You can add an optional Other text box to Multiple Choice Questions by checking the option in the question settings. You will find the option at the bottom of the question choices in the Rows settings panel.
On the form template, place the cursor where you want to insert the control. If the Controls task pane is not visible, click More Controls on the Insert menu, or press ALT+I, C. Under Insert controls, click Text Box.
