To get checkboxes to work, modify the field to Yes/No in design view. Save in datasheet view, confirm data loss message. Checkboxes now visible for members with Yes. Open form, delete existing field, insert merge field in mailings tab. Follow steps in provided link, do Alt + F9 to view field.
