You can add a custom field to a template. Contacts menu Settings Template Add Field.
To add a contact: The iPhone gives you different ways to save contact information from the Phone app: Tap Contacts, then tap the + button in the top-right corner. Tap Recents, then tap the Info button next to the desired number. From here, you can create a new contact or add the number to an existing contact.
Open the Contacts app and tap the Lists link in the upper-left corner of your main Contacts list. Select Add List, enter a name, and tap Done. You can add people to a list by tapping one from the Lists screen and selecting Add Contacts on the empty page. Select the individual contacts you want to add and tap Done.
