5. In Section 3, provide the total state and local tax shown on your W-2/1099 forms. If applicable, combine totals from both forms.
6. Complete Sections 3a and 3b by entering the total withholding tax paid and any eligible tax-exempt credits respectively.
7. Calculate the amount due in Section 4 by subtracting lines 3a and 3b from line 3. If this is zero or greater, enter it here; otherwise, proceed to line 5.
8. If there’s an overpayment (line 5), enter it as a positive number. Specify how much you want applied as a credit or refunded in Sections 6 and 7.
9. Finally, sign and date the form to declare its accuracy before submission.
