Next, move to the Employee or Contractor Information section. Input the Social Security Number (SSN) or check the box if using FEIN for a contractor. Fill in the first name, middle initial, last name, address, city, state, zip code, and date of hire.
Indicate whether this is a contractor by selecting 'Yes' or 'No'. If applicable, provide the date payments will begin for contractors and specify how long they will be performing services.
Review all entered information for accuracy. Ensure that all required fields are completed as reports will not be processed if information is missing.
