On line 3, select the employment tax period for which you are claiming the credit by checking the relevant year and quarter.
Complete lines 4a and 5a by entering total qualified first-year wages paid to certified veterans, then calculate and enter the respective amounts on lines 4b and 5b.
Add lines 4b and 5b together on line 6, then follow instructions for lines 7 through 12 based on your previous filings and current claims.
