In the Tax Matters section, check the box if it applies to 'all tax matters' for the specified tax period. Otherwise, list each tax type along with its corresponding Ohio account number and tax period.
Set an expiration date for this declaration; it can be no more than three years from today. If left blank, it will remain valid until revoked.
Complete the Restrictions section by confirming your eligibility as a representative and signing where indicated. Ensure all required signatures are provided before submission.
