  5. Indicate whether you currently have access to PeopleSoft systems by selecting 'Yes' or 'No'. If you are a new user, remember to sign the Computing Awareness & Data Security Compliance Statement and attach it to this form.
  6. Briefly describe why you need access to HR data in the provided space. Then check all types of HR access needed from the list.
  7. If requesting View/Inquiry or Timekeeping access, list the relevant DEPT IDs or GROUP IDs as required.
  8. Obtain necessary signatures from your department head and provide your own signature acknowledging responsibility for data confidentiality.
  9. Finally, send the completed form to the HR Data Custodian at the specified address. Note that faxed copies are not acceptable.
