In section 11, indicate whether you are exporting or importing currency by completing either 11a or 11b. Provide details about the departure and arrival locations.
If applicable, complete section 12 regarding any mailed or shipped currency. Enter dates and methods of shipment accurately.
Move to Part II to provide information about the person or business on whose behalf the transaction is conducted.
In Part III, specify the type and amount of currency involved. If other than U.S. currency is used, complete blocks A and B accordingly.
Finally, sign and date the report in Part IV to certify its accuracy before submission.
