Click ‘Get Form’ to open the Alabama PVQ Comptroller in our editor.
Begin by entering the 'Date Prepared' and 'Prepared By' fields. Ensure accuracy as these details are crucial for processing.
Fill in the 'Agency' and 'Transaction Code' sections. The transaction code should be marked as 'PVQ'.
Indicate the number of documents being submitted in the '# of Docs' field, followed by specifying the type of document.
Sequentially enter each 'Voucher Number' in numerical order, ensuring that they correspond correctly to your records.
Complete the 'Batch Number' and 'Batch Total', noting that the batch number consists of your agency number plus the last three digits of voucher one.
Input the 'Vendor Number', which should be an 11-digit figure, followed by filling out the total for each voucher listed.
Finally, remember to attach two adding machine tapes at the back of your transmittal—one for voucher line totals and another for overall voucher totals.
