Begin by filling out the project details in sections 1a to 1c, including the project name, number, and contractor information.
In sections 2a to 2d, enter the employee's full name, phone number, home address, and verify their identification.
Proceed to sections 3a to 4c. Ask the employee about their job duration, last working day, hours worked, hourly rate of pay, and fringe benefits. Ensure accuracy by checking any available pay stubs.
For sections 5 to 7, document the employee's job classification(s), duties performed, and tools or equipment used during their work.
Complete sections 8 to 12b by confirming if the employee is an apprentice or trainee and whether they are paid for all hours worked. Collect their signature and date.
Finally, fill out sections 13 to 15c with observations made during the interview and ensure that both the interviewer’s name and signature are recorded.
Review section 16 for any remarks regarding payroll examination discrepancies before finalizing your form.
