4. List all leaders attending. If space is insufficient, attach an additional list. Repeat this for Parent/Guardian attendees.
5. Fill in details about Volunteer Helpers or Resource Persons attending. Ensure PRC verification is noted where applicable.
6. Complete the location details of the camp or outing including Facility Name, Phone number, Address, Description of Program, Route Plan, Mode of Transportation, and Distance to Travel.
7. Review the Safety Checklist thoroughly. Check off each box as conditions are met before signing at the bottom of the form.
8. Once completed and approved by the Group Commissioner, submit a signed copy along with any related documents to your Administrative Centre for filing.
