2. Begin by filling out the 'Receiving Broker Information' section. Enter your broker name, agency name, address, city, state/provincial code, zip/postal code, country, email, primary phone, cell phone, and fax number.
3. Next, complete the 'Sending Broker Information' section with the same details for the sending broker.
4. In the 'Client Information' section, provide the client's name and current address details including city, state/provincial code, zip/postal code, and country.
