Throw a thon form 2026

1. Click ‘Get Form’ to open the throw a thon form in our editor.
2. Begin by entering the player's name in the designated field at the top of the form.
3. Next, input the donation amount for each made free throw. This could be any amount such as .25, .50, or $1.
4. For each patron, fill in their name and record the total free throws made by the player. You can list up to 20 patrons.
5. After completing all patron entries, calculate and enter the total donation based on the number of free throws made multiplied by the donation per free throw.
6. Finally, ensure that both the player and coach sign and date the form at the bottom to certify completion.
