Animals entering the United States may be subject to regulation by APHIS and other Federal agencies. Depending on your destination State, your pet may need to meet State entry requirements in addition to Federal entry requirements. Pet owners are responsible for meeting all Federal and State requirements.
You need a filled out CDC form, microchip number and rabies vaccination documentation, also the pet needs to be recently wormed. I flew back with my dog a few weeks ago, and all of it, including the screwworm documentation was needed.
Certification Requirements However, most countries require that your pet be checked by a federally-accredited veterinarian and that a United States Interstate and International Certificate for Health Examination for Small Animals (7001 USDA-APHIS Form ) be issued by that veterinarian and endorsed by the USDA.
When traveling to foreign countries, you will need an International Certificate of Veterinary Inspection (health certificate). You may also need a permit and have to fill out a form. Information about pet passports to foreign countries can be found at Pet Travel.

Dogs must appear healthy upon arrival. Dogs must be at least 6 months old at time of entry or return to the U.S. Dogs must have a microchip that can be detected with a universal scanner to identify them. The microchip must be implanted before getting the rabies vaccine, or else the vaccine will be invalid.
