3. Move to Section B to enter retailer information. Fill in the trade name of the dealership, complete address and phone number, sales tax registration number, dealer number, and purchaser's name and address as shown on sale documents.
4. In Section C, the purchaser must declare their information. Enter your name, driver's license number along with state of issuance and residential address. Ensure all declarations are accurate before signing.
