GA modifier indicates that an Advance Beneficiary Notice (ABN) is on file and allows the provider to bill the patient if not covered by Medicare. Use of this modifier ensures that upon denial, Medicare will automatically assign the beneficiary liability.
An ABN gives you the opportunity to accept or refuse the items or services and protects you from unexpected financial liability in cases where Medicare denies payment. It also offers you the right to appeal Medicares decision.
The GA modifier is defined as waiver of liability statement issued as required by payer policy. Use the GA modifier to report an ABN was provided to the beneficiary indicating the likelihood of denial of the service as being not reasonable and necessary under Medicare guidelines.
If you have Original Medicare, your doctor, other health care provider, or supplier may give you a written notice if they think Medicare wont pay for the items or services youll get. This notice is called an Advance Beneficiary Notice of Non-coverage, or ABN.
Modifier -GA notifies Medicare that you obtained a signed ABN. Without the modifier, Medicare will notify the patient that they are not responsible for payment of the service; the physician should have provided a statement of possible noncoverage.
