Proof of ownership in the form of a New Jersey insurance card or registration card showing the connection of the owner to the Vehicle Identification number. The card(s) may be current or expired. OS/SS-52 Application for Duplicate Title Form.
Sellers of residential property must complete the Disclosure Statement (questions 1 through 108). A copy of the Disclosure Statement is available here; and. All sellers of real property, both residential and non-residential, must complete the Flood Risk Addendum to the Disclosure Statement (questions 109 through 117).
A copy of a deed or probated will indicating ownership is acceptable proof of legal ownership of such property.
