4. Next, move to the Should be Paid Section. Document all hours that reflect what you should be paid for that week. Ensure these hours total 40.00.
5. If applicable, complete the Higher-Level or Dual Appointment Section by checking the appropriate box and entering relevant codes and levels.
6. Use the Remarks Section for any supporting statements or grievance details if necessary.
7. Finally, ensure both you and your authorizer sign and date at the end of the form before submitting it through our platform.
